In short, yes! Manual therapy has an immediate but very short lasting effect on your pain. How you move habitually and what you do on a daily basis has contributed to your pain and conversely, the therapy for injury prevention and rehabilitation that you do on a daily basis in your own environment is a more effective, long term strategy in controlling your symptoms.
If you live in areas with no Physiotherapy access, virtual Physiotherapy can be very convenient and effective in treating your pain.
If you need a second opinion, Zoom can be a very efficient way of getting a second opinion.
If you are a Physiotherapist working remotely, after you get your patient's consent, we can Zoom with your patient present to discuss progression or complex and challenging presentations.
No, not all patient presentations are suitable for virtual physiotherapy.
For example, if you have had recent surgery, frailty that require support to stand, fracture or trauma, I advise you to consult a local physiotherapist who can be right next to you as your balance is challenged during treatment.
If you have a recent neurological injury either to the brain (stroke, traumatic head injury) or spinal cord, it is likely you need intensive face to face team support from physiotherapists, occupational therapists, and speech and language pathologists all working very closely together to ensure excellent outcome.
If you sincerely and strongly believe that you need manual therapy to get better then virtual Physiotherapy may not be for you
What I have seen repeatedly in clinic is that patients who start with poorer function but are highly motivated often recover to a higher level than those who have better function but are poorly motivated.
Our body is truly amazing and our brains have wonderful plasticity, meaning that if our comprehension is not compromised, we are always able to learn new ways of moving by building new neural pathways and motor patterns. We do this by redirecting the attention of the brain by specific education into your condition and presentation, sensory input, therapeutic stretches and strengthening and breath retraining.
I only work with individuals who are motivated to improve their movement. I use this framework to effect change in my patients
(1) I get a thorough history from you to fully understand why your symptoms have presented and the impact it has on your life.
(2) I assess your range and movement and look at your functional abilities
(2) We agree on the goals to be met.
(3) I start with damage control. The immediate goal is to stop the pain cycle.
(4) We start working on strengthening stability muscles
(5) As you start to be able to control your symptoms, we will start with more challenging movement.
(6) Throughout this process, I will use 3D anatomical models to teach you.
(7) Discharge when your goals are met. And unlike discharge from a traditional clinic, you won't feel like you're missing something because over our sessions, you would have acquired the skills and confidence to self manage effectively.
The most important measure is YOU! Isn't it far more important for YOU to tell me that you have improved than for me and my ruler to tell you that you have improved?
You live with your symptoms everyday and while we clinicians call it subjective, to me it is more important than what I measure. Subjective is what brought you to seek help in the first place.
